Our goal at Orbit is to make ordering equipment as easy as possible allowing you to focus on the things important in your industry. Though insurance guidelines can be stringent and time consuming, you should never have to worry about babysitting an order or a patient discharge. Following our simple process makes ordering DME a breeze.
Submit a referral via fax:
- Download our DME Discharge Form
- Fax the filled out form along with the patient’s face sheet and most recent chart notes to 800-430-0255
Submit a referral via our website:
- Click on Submit a Referral
- Fill out the necessary information
- Upload the patient’s face sheet and chart notes
- Submit
